Evolving Q-wave infarction with a paced rhythm.

I. There is a paced ventricular rhythm present. This makes interpretation difficult as it is hard to extrapolate ischemic changes from a paced rhythm.

But in this case there is obvioius ST elevation in leads V1-V4 showing acute anterior infarction.

II. This second tracing, taken several hours later, nicely illustrates the evolution of a Q-wave infarction.
Q waves in V1-V4 show the presence of evolving anterior infarction. Do not misinterpret the pacer spikes for R waves!
