History: These recording are from a 47 year old computer technician with a one year history of painful skin lesions on his palms and soles. Each episode lasted 1-2 weeks. The patient had multiple embolic events which included episodes of bilateral foot ischemic and three episodes of right radial embolism. The patient had shortness of breath bending forward or sitting up, which resolved when lying back. Laboratory data: Hgb 119, MCV 77.4, RDW 12.3, PT 14, PTT 36.5 ,INR 1.3, Na 133, K 3.8, Cl 86, Cr 81, BUN 4.6, Glucose 6.0, ANA negative, RF Neg. Physical: Bp 130/80, P = 80, regular 3-4 cm skin lesions on right foot ( medial, dorsal) lasting 2 weeks, slight dusky, erythematous with tenderness. Split S1, diastolic sound when leaning forward. Pathology: A 2 .3 cm left atrial myoma was found attached to the left side of foramen ovale. The myxoma was resected and the septum reconstructed with a pericardial patch |
