The patient was initially in sinus rhythm when the pacemaker was implanted but then converted to atrial flutter/fibrillation.
The pacemaker is attempting to follow the fast atrial rate but is limited by the upper rate limit which was set at 120/min.
The pacemaker was switched to VVI mode as sinus rhythm could not be maintained.
